Anyone who knows my story, knows that my husband starting playing before I did, so I was introduced into the game extensively by watching over his shoulder.
There was NO WAY I was going to sweat, let alone run around in those godawful OJ's ... so we deposited $25 for me to start.
I got basic stuff, full Paladin, a mk1, an amp, etc. etc.
We deposited a few times after that, and didn't feel any remorse as we deposited to have fun, and figured it would be part of our entertainment part of the budget.
Since the tiger arms, we haven't had to deposit much at all, and I got that as my first uber loot, 3 months into the game.
I don't think I will ever withdraw, as I enjoy the game too much .. but don't quote me on that.
Now, if I were to hit some HUUUUUGGEEE hof .. there's the possibility that I would withdraw, but when you work it out to $$$ & the fees on the ATM Card & the limits on the withdrawals per day .. sometimes it's not even worth it in the end.
But .. who knows.

As for the loot pool .. I think that comes mostly from Ad's, Mall sales... etc. etc.
